Nursing Positions
- RN Referral/Admission Nurses Meet with patients and families to provide information about quality end-of-life care provided by Hospice and complete the initial admission process in a compassionate, concise manner
- RN Staff Nurses Provide physical assessments and symptom management to meet patient needs and coordinate care with the interdisciplinary team
- RN and LPN Concentrated Care Nurses Provide one-to-one bedside care of patients for symptom control and support of patients and families in patients homes and facilities under the direction of the primary RN
- LPN Office Nurses Handle phone calls from patients, families, staff and other providers and process required forms and information
- LPN Visit Nurses Make daily visits to patients providing nursing care and support under the direction of the primary RN staff nurse
- CNA (certified nurse assistants) Provide personal care needs for patients such as bathing and daily cares
Non-medical Support Positions
- Human Resource Director Provides decisive leadership and management, performs strategic planning, and is a part of our executive team, reporting to the executive director.
- Clinical Manager Manages the daily operations and resources for the clinical team to ensure optimum quality patient/family care and develops staff through training, mentoring and coaching.
- Administrative Assistant - Referral Coordinates referral requests with interdisciplinary team, under the direction of the referral admit coordinator. Excellent telephone skills required, this position will also provide referral/admission data entry and other daily administrative duties.
- Licensed Social Workers Help patients and family members cope with life-threatening illnesses and ascertain their impact on the family; they foster open communication and find ways to fulfill individual and family needs while providing guidance about loss and grief
- Chaplains Provides spiritual care to patients and families. Our chaplains are ordered, ordained or licensed by a recognized denomination.
- Bereavement Specialists Licensed social workers and/or licensed counselors who provide bereavement care for the family following a patient’s death.
- Development Officer Develops relationships with and secures financial support from individuals, businesses, churches, and organizations who have the potential to contribute financially to the mission of providing comfort and support at the end of life's journey.
- Medical Records Clerk Assists patient care staff with maintaining accurate medical records to ensure excellent customer service to our patients and their families. Also performs data entry tasks and makes record information available as needed.
- Support Services Staff Provide clerical and administrative services; public relations; volunteer coordination services; finance and information technology services.
