Who provides hospice care?
Each patient and family benefits from a team of professionals working together to best meet their needs. The team consists of a doctor, nurse, certified nursing assistant, social worker, chaplain and volunteer. Hospice care is coordinated among individual team members and the family. Hospice bereavement specialists are also available to help deal with the anticipated loss and follow-up grief support for the family.
